**Payments for services are due at the time of service. To receive the yearly discount, payment must be collected in a single payment at the time of enrollment or yearly renewal.
No refunds of premiums will be issued at any time if participant decides not to utilize dental plan.


This is a dental discount plan and is NOT dental insurance and cannot be combined with any other dental insurance or finance programs such as Care Credit. This dental discount plan is limited to the office of John S. Faerber D.D.S.. It may also not be combined with other discounts or promotions. Should there be dental treatment needed following any type of injury where a lawsuit & therefore outside medical care, disability or workman’s comp type insurance is involved, this discounted plan cannot be used.

  • 2 (per year) continuing care Oral Exams with hygiene visits
  • 2 cleanings (every 6 months) by Dental Hygienists (not including additional periodontal services which may be necessary)
  • Diagnostic Digital X-Rays one complete annual required x-ray series (bitewings or full mouth series, panoramic every 3-5 years)
